Overview

The Transportation Coordinator oversees all PACE participant transportation to and from the PACE Center as well as to and from all approved appointments outside of the PACE Center. Coordination of drivers, vans and any necessary support staff is supported by the Transportation Coordinator who will work closely with the interdisciplinary team to communicate any pertinent updates or planning. Additionally, this position is responsible for the safe and efficient operation of UHC owned transportation passenger vehicles.

Review of all owned transportation vehicles is scheduled and audited by this role. The performance of a mechanical check prior to vehicle operation is expected each day. Work involves assisting passengers as they step on and off the vehicle and also includes assisting wheelchair bound patients. Work involves considerable contact with the public and requires the exercise of tact, judgment and diplomacy.

Additionally, all missed trips, transportation breakdowns and rescheduled trips are reviewed and corrected through this role. Finally, the Transportation Coordinator ensures a smooth and well- connected participant flow with the PACE center supporting all recreation therapy, dining, rehabilitation and health center visits.

Responsibilities

PERFORMANCE AREA 1:

* Provides transportation services members from/to home to/from day health center sites and/or medical appointments according to assigned route/appointment schedule or as may be directed by the Dispatcher.

* Adheres to all agency, department, and State rules and regulations related to all aspects of providing safe and comfortable transportation services to members. This includes but is not limited to, use of gait belts, proper loading and unloading of passengers, proper wheelchair securement, safe and lawful driving, and assuring that members are not left unattended at any time.

* Functions as a member of the interdisciplinary team.

* Reports any changes in the member's home or behavior to the Transportation Supervisor or to other appropriate staff person as procedures indicate.

* Reports any vehicle safety/maintenance issues to the Transportation Supervisor as they arise.

* Follows established departmental van cleaning and refueling schedules.

* Completes route sheets on a daily basis including times of pickups and deliveries and turning in same to assigned Transportation Supervisor.

* Provides pick-up and delivery services, including mail, meals, medications, incontinent products, nourishments and Durable Medical Equipment.

* Attends staff meetings and agency in-services as required.

* Follows procedure for reporting accidents immediately after such incidents occur, whether vehicular or personal.

* Protects privacy and maintains confidentiality of employee, member and sensitive agency information.

* Supports agency's mission and values.

* Performs related duties as assigned.

* Participates as a member of the interdisciplinary team, facilitating on-going communication regarding transportation services; maintains regular attendance and represents the Transportation Department at team and other staff meetings and in-services.

* Oversees the coordination of transportation for participant supplements and medication deliveries.

* Oversees coverage due to driver absences.

* Problem solves transportation-scheduling issues and notifies other appropriate staff of issues.

* Assists in investigating complaints and completes appropriate documentation.

* Assists in monitoring safety and maintenance of vehicles and equipment.

* Conducts monthly vehicle safety inspections in accordance with Transportation policies.

* Utilizes general office equipment as required for daily operations, including computer system, two-way radio, telephone, fax machine and photocopier.

* Oversees the maintenance of site-based Transportation computer database.

* Provides summary reports to administration per schedule and as requested.

* Protects privacy and maintains confidentiality of sensitive employee, participant and agency information.

* Promotes and maintains a safe working environment. Follows agency Safety Policies and Procedures.
