** Job Summary
** The Transportation Coordinator is responsible for coordinating and dispatching transportation routes. This includes both in-house and outside appointments. Transportation Coordinator’s will also be responsible for participating in Inter Disciplinary Team (IDT) and Care Planning meetings.
** Essential Job Functions
** Duties include, but not limited to:
* Participate as a member of the Inter Disciplinary Team and Care Planning meetings, and report changes in participants’ baseline status to appropriate staff.
* Collaborate with applicable departments to coordinate transportation for all participants scheduled clinic and outside provider appointments, day center and outings.
* Involved in the implementation of Quality Improvement activities.
* Develop and maintain transportation routes manually and with transportation software.
* Dispatch and communicate new routes to drivers with their participant routes.
* Prepare transportation metrics and driver performance and safety reporting for Management, as needed.
* Prepare monthly transportation ride count reporting.
* Work with Transportation Assistant(s) and Drivers to ensure vehicle maintenance and repairs.
* Work with other departments to coordinate additional routes, meal delivery, DME and/or care packages.
* Attend and participate in staff meetings, in-services, projects, and committees as assigned.
* May be asked to drive a company vehicle and complete routes to provide support for the department.
* Understands and navigates the Electronic Health Records system.
* Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.
* Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
* Be flexible in the schedule of hours worked.
* May require use of personal vehicle.## Qualifications
