- Evaluate patients in their homes and create individualized treatment plans.
- Improve mobility, strength, balance, endurance, and functional independence.
- Educate patients and caregivers to support long-term recovery.
- Collaborate with nurses, physicians, occupational therapists, speech therapists, and other members of the interdisciplinary team.
- Monitor progress and adjust treatment plans to maximize outcomes.
- Complete accurate and timely documentation.
- Active Minnesota Physical Therapist (PT) license.
- Excellent clinical reasoning and communication skills.
- Passion for helping patients achieve meaningful, lasting outcomes.
- Ability to work independently in a field-based setting.
- Valid driver's license and reliable transportation.
