Job Overview

Ebenezer Senior Living is looking for a LPN - Licensed Practical Nurse to join our senior living team at Willows Bend in Fridley, MN! This position performs a variety of professional nursing duties in compliance with Class F home care regulations and follows established policies and procedures.

This nurse schedule includes:

  • On Call/Casual
  • Day and Evening shifts available
  • To maintain Casual status, employees must be willing to be available to work a minimum of three (3) shifts per month (two shifts must be weekend shifts) and one (1) Holiday shift per year. The weekend starts with the beginning of the night shift on Friday and ends with the beginning of the night shift on Sunday

Responsibilities
  • Provides professional nursing care:
    • Consults with Registered Nurse (RN) regarding resident condition and treatments.
    • Promptly transcribes and implements practitioners’ orders.
    • Orders medication and supplies in a timely manner
    • Assists RN in adherence to medication administration procedures.
    • Assembles and operates medical equipment used for resident care.
    • Reports changes in resident status to the RN.
    • Initiates emergency treatment as needed. Reports status to RN.
    • Performs skilled nursing care per individualized care plan under the direction of RN.
    • Carries out proper charting practices and procedures
    • Assists in completing supervisory/monitoring visits.
    • Performs any other duties as assigned or delegated by RN.
  • Monitors work and performance of unlicensed staff:
    • Meets with staff to coordinate care and address care issues.
    • Assists RN in implementing the work assignment of unlicensed staff within established guidelines.
    • Assists RN in monitoring the performance of unlicensed staff.
    • Assist in training and orientation of employees as directed by RN.
    • Assists and supports unlicensed staff as needed.
    • Assists RN in staff assignment/scheduling activities that support efficient and effective utilization of staff as needed.
  • Maintains current observation and documentation of tenant/resident health status
    • Supports RN to complete initial and on-going assessments per Minnesota home care regulations and Nurse Practice Act.
    • Assists RN to assure that tenants/residents have up-to-date service agreements and care plans.
    • Maintains clinical charts and progress notes using computerized documentation system.
    • Communicates changes in resident conditions to RN, practitioners, and other relevant individuals.
  • Performs other duties as assigned
Required Qualifications
  • Less than 1 year of previous experience
  • One to three years of previous experience
  • Ability to use technology in the form of computers, handheld devices, cell phone & fax machine
  • Proficient in Eldermark computer documentation software
  • Basic Life Support (American Heart Assoc or Red Cross) Current Basic Life Support (BLS) for Health Care Provider from American Heart Association (AHA) or American Red Cross; accepted certifications as follows
    -AHA: BLS for Healthcare Providers (CPR & AED) Program or BLS Provider - BLS Instructor. American Red Cross: CPR/AED for Professional Rescuers and/or Healthcare Providers, Life Guarding First Aid/CPR/AED. BLS is required upon hire and/or new hire must show proof that they are currently enrolled in a BLS class to renew or obtain their certification. BLS certification must be obtained within three-weeks of hire date and before nurse can perform independent practice.
