Key Responsibilities:
  • Medication Management: Oversee and adjust patient medication plans to ensure safety and efficacy.

    Education:

    Empower patients with the knowledge they need to manage their medications effectively and safely.

  • Medication Reconciliation: Conduct detailed reviews during patient transitions to ensure continuity of care.
  • Regulatory Compliance: Stay current with and adhere to all pharmacy laws, regulations, and safety standards.
  • Quality Improvement: Lead and participate in initiatives that enhance medication safety and drive better patient outcomes.
Qualifications:
  • PharmD or RPh from an accredited school.
  • 1+ years of Clinical Pharmacist experience.
  • Active and valid pharmacy license.
  • Strong knowledge of DEA and FDA health regulations.
  • Exceptional attention to detail, analytical thinking, and communication skills.
  • A personable, customer-focused approach to patient care.
