Home Health Physical Therapist

• Administer physical therapy to patients on an intermittent basis in their place of residence
• Provide physician-prescribed physical therapy
• Improve or minimize residual physical disabilities
• Return individuals to an optimum and productive level within their capabilities
• Participate with home care personnel in patient care planning
• Direct and supervise personnel as required
• Complete initial assessments, plans of care, maintenance program development and modifications, and reassessments every 30 days
• Provide quality home healthcare that helps patients remain independent at home

• Graduate with a Bachelor’s, Master’s, or Doctorate in physical therapy from an approved physical therapist curriculum
• Currently licensed in the state in which practicing
• Two (2) years’ experience preferred in a home health setting
• Knowledgeable and experienced with Medicare OASIS documentation
• Passion to serve and help others live their best lives possible

Demonstrates expertise in administering physical therapy in home health settings, including conducting assessments, developing care plans, and ensuring compliance with Medicare OASIS documentation. Possesses strong leadership skills to direct and supervise personnel while fostering patient independence.
