Care Transition Specialist – Case Management

Work location: Texas Health Frisco, 12400 N Dallas Parkway, Frisco, TX 75033
Work hours: Full time;
Monday - Friday; 8:30 – 5:00p

- H.S. Diploma or Equivalent required
- Three years’ relevant experience, preferably in a healthcare or service environment required
- Patient Care/ Customer Service / Healthcare experience strongly preferred
- Responsible for coordinating the clerical and administrative processes for entity care transitions management
- Responsible for managing deparmental mail box, phone lines and/or fax
- Supports transition planning activities as needed under the direction of care transition managers
- Calls vendors, physician offices, clinics, etc
- Faxes (electronic and traditional) documents as needed
- Makes copies or gathers other data as needed
- Collect and enter data as needed
- Completes other tasks and projects assigned
