Job Responsibilities:

Conduct thorough patient assessments and develop individualized care plans.

Deliver skilled nursing care according to physician orders and care protocols.

Educate patients and families on health maintenance, disease prevention, and care instructions.

Maintain accurate documentation in compliance with agency and regulatory standards.

Collaborate with the Bright Star Care team and other healthcare professionals to ensure optimal patient outcomes.

Qualifications:

Active RN license in the state of practice.

Previous experience in home health or acute care settings is preferred.

Strong clinical, organizational, and communication skills.

A passion for providing compassionate, high-quality care.
