The Orthopedic Care Coordinator is responsible for providing and/or coordinates educational and process improvement activities for both staff and patients. Acts as a community liaison on behalf of the program and hospital. Directly involved in the marketing/promotion of services provided.
What You Will Do In This Role:
- Oversees the overall care of orthopedic/Joint patients following a holistic and interdisciplinary approach in collaboration with medical staff, patients/family members and other members of the health care team
- Serve as a leader for the overall program management, process improvement plans, patient satisfaction plans and communicate goals and status to staff and other team members
- Coordinate and drive the monthly performance Interdisciplinary meeting. Collect, analysis and report the quality metrics data with team members and plan interventions, ensure appropriate action /process is in place to achieve great outcome/goals
- Responsible for coordinating and providing education for patients and families before surgery (Pre-op classes) and before discharge (discharge education). Coordinate teaching materials for patients/families and physician’s office
- Collaborate and facilitate staff education on various aspects of orthopedic patient care to assist the team following best practices/clinical practice guidelines in align with current process/protocol
- Make daily clinical rounds on patients, assess the therapy and other need of patients from admission through discharge and coordinate care needs with other team members
- Daily rounding with physicians, provide update on patient progress/ any challenges, need for any interventions to improve the patient care, treatments and overall experience
- Concurrent chart reviews are performed to obtain data, analysis documentation of clinical components that meets the joint protocol/standards
- Collaborate with other disciplines ensuring the discharge needs are met including discharge education, after discharge therapy, HHC, DME’s, follow up appointment etc.
- Collaborate/create measurement system for tracking surgical outcomes, patient satisfaction, identify areas that require improvement
- Oversees, plan and develop formal and informal educational material for patients/families and ensure educational needs are met. Coordinate teaching materials with physician’s office
- Act as a Liaison to the patient/family, physicians/office staff and other team members involved with the orthopedic patient care/treatment
- Coordinate patient post discharge calls to ensure continuum of care pathways are followed/met
- Coordinate/collaborate community outreach programs to involve the community with educational opportunities.
- Coordinate all the aspects required for recertification of joint program with Joint Commission and any other accreditation bodies
What qualifications you will need:
- Graduate of an accredited school of Professional nursing
- BSN is required
- Advanced Practice Nursing/Masters prepared preferred
- Current RN licensed in the State of TX or compact license
- Orthopedic Nurse Certified
- Must have a minimum of 5 years recent clinical nursing in area of orthopedics
- Recent experience in Orthopedic outcomes research and intervention is desirable
- Current BLS issued by the American Heart Association or the American Red Cross
- Current ACLS preferred, issued by the American Red Cross or the American Heart Association
