Care Coordinator, Embedded
Full‑time embedded Care Coordinator located in the Front Royal, Virginia area. Provides on‑site support within partnered long‑term care facilities, working alongside Ennoble Care providers in a family‑practice‑style model of care.
Key Responsibilities- Coordinate care for long‑term care residents using Ennoble Care’s EMR.
- Maintain regular, proactive on‑site engagement with residents to support care plans, follow‑ups, and care transitions.
- Collaborate closely with Ennoble Care providers within the long‑term care setting.
- Serve as a key liaison between providers, facility staff, residents, and families.
- Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner.
- Accurately document patient interactions and care coordination activities.
- Follow established workflows, documentation standards, and escalation protocols.
- Contribute positively to team collaboration and strong facility partnerships.
- Legally authorized to work in the United States of America.
- Active, unrestricted clinical certification or licensure, such as:
- Patient Care Technician (PCT)
- Certified Nursing Assistant (CNA)
- Medical Assistant (MA) / Certified Medical Assistant (CMA)
- Licensed Practical Nurse (LPN)
- Registered Nurse (RN)
- Or comparable credential
- Minimum 1 year of patient‑facing experience in a long‑term care, skilled nursing, or similar clinical setting.
- Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person.
- Cultivate and maintain positive, approachable relationships with residents, families, providers, and facility staff, while demonstrating a consistent, dependable presence within the long‑term care setting.
- Well‑developed organizational and time‑management skills, paired with an optimistic approach and initiative‑driven mindset.
- Strong working knowledge of healthcare terminology and care coordination principles.
- Proficiency with Microsoft 365, including Outlook, Word, and Excel.
- Ability to work full‑time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST (or contingent on facility hours) on site in the Reston, Virginia area with a primary focus on the Front Royal vicinity.
- No weekends.
- No on‑call.
- Daytime hours.
- Seven (7) company‑paid holidays annually.
- Transportation criteria:
- Valid, unencumbered driver’s license.
- Consistent, reliable transportation to fulfill the duties of this position.
- Current auto insurance, with the ability to provide proof.
- Travel between facilities may be required.
- Ability to pass a background check and drug screen.
