Job Summary:
As a Licensed Practical Nurse (LPN) at Frostburg Rehab Center, you will play a vital role in delivering safe, timely, and effective nursing care to our residents under the direction of an RN or physician. You will also monitor patients' health, administer medications, and provide wound care and other treatments. Our team-oriented environment encourages collaboration with interdisciplinary teams to develop and update individualized care plans, support residents and their families with education, comfort, and reassurance, and maintain accurate documentation and comply with regulatory standards.
Responsibilities:
Deliver safe, timely, and effective nursing care to residents under the direction of an RN or physician
Monitor patients' health, administer medications, and provide wound care and other treatments
Collaborate with interdisciplinary teams to develop and update individualized care plans
Support residents and their families with education, comfort, and reassurance
Maintain accurate documentation and comply with regulatory standards
Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team
Observes safety policies and reports any safety concerns
Demonstrates respect of resident rights and dignity
Fulfills the role of charge nurse when assigned
Supervise and support Geriatric Nursing Assistants (GNAs) in the delivery of care
Maintain a clean, safe, and therapeutic environment for residents
Ensure compliance with infection control, safety, and HIPAA policies
Participate in quality assurance activities and staff meetings
Requirements:
Active LPN license in Maryland or Multi-state
Experience in long-term care or skilled nursing preferred - new grads welcome!
Strong clinical skills, attention to detail, and a heart for service
