Position Overview

Horizon Home Health & Hospice West is seeking Social Workers in the Fruitland, Idaho area. The role involves supporting patients and families through psychosocial assessment, counseling, and coordination of community resources within a home health/hospice setting.

Qualifications

MSW or equivalent license in the State of Idaho
Valid driver’s license
Prior home health or hospice experience preferred

Responsibilities

Assess psychosocial status of patients and families related to terminal illness and communicate findings to the interdisciplinary team.

Carry out social evaluations and plan interventions based on findings.

Provide counseling to patients and caregivers regarding stress and coping.

Maintain accurate clinical records for all patients.

Offer information and referral services for practical and environmental needs.

Serve as liaison between patients/families and community agencies.

Collaborate with organizational personnel to support patient care.

Develop and maintain contracts with public and private agencies.

Assist the volunteer coordinator in establishing volunteer services for patients and families.

Participate in individualized care plans and attend interdisciplinary group meetings.

Adhere to documentation and care procedures and professional conduct standards.

Assist physicians and team members with understanding social and emotional factors related to health problems and end‑of‑life issues.

Participate in quality assessment, performance improvement teams and activities.

Perform other duties delegated by the clinical supervisor.
