The LVN supports the Director of Nursing, Case Management and the nursing staff in the delivery of health care by performing patient care assessments, and a variety of medically related tasks. The LVN is a licensed nurse who works in an Home Care Setting to provide nursing services.
Responsibilities- Providing nurse visits to home health patients (home visits)
- Care for ill, injured, or convalescing patients by implementing plan of care initiated by RN
- Provide basic patient care and treatment; document consistently with the plan of care
- Communication with patients to schedule visits
- Educating the patient how to take care during non-visit days
- Demonstrate knowledge of regulatory requirements
Our patients need care 7 days a week. While you have flexibility in your scheduling, we ask that you be able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).
Experience and Requirements- Visiting LVN for Home Health: 1 year (Required)
- Nursing: 1 year (Required)
- Current CA LVN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Preferred)
