Our patients need care 7 days a week. You must be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer).

Work Area:
City close to you, with a group of patients who need care for that day.

• Travel to patient’s home to administer services and help maintain independence.
• Administer at‑home IVs, change dressings, clean wounds, perform in‑depth assessments, and oversee patient treatment plans.
• Update doctors and the agency about patient health via Synergy Software.
• Perform an initial, comprehensive assessment; document and upload to Agency Portal.
• Obtain and document physician orders.
• In consultation with the assigned Clinical Team Manager, initiate, re‑evaluate, and revise the plan of care regularly.
• Coordinate care with the Clinical Team Manager, physician, and other home‑care team members, informing them of significant changes in the patient’s condition and needs.
• Perform re‑certifications, resumption of care, transfers, and discharges as requested.
• Complete and submit all related documentation.
• Assess need for services of other team members (PT, OT, ST, MSW, HHA).
• Perform Home Health Aide supervisory visits per regulatory requirements.
• Demonstrate knowledge of regulatory requirements.

• Visiting RN for Home Health experience: at least 1 year (Required).
• General nursing experience: at least 1 year (Required).
• Current California RN license (Required).
• BLS certification from the American Heart Association or American Red Cross (Required).
• NSO Nursing License Insurance (Required).
• IV/Phlebotomy certification (Required).
