Gentiva Hospice is seeking a dedicated RN Case Manager to lead patient care in home settings across Fuquay-Varina, NC. You will assess, plan, implement, and document individualized hospice care, coordinating with physicians, LPNs, CNAs and social workers.
You will educate families, manage symptoms, and ensure compassionate support within regulatory guidelines. A strong nursing background and CPR are required.
