Patient Retention Specialist
A professional who assists in the prevention of acute care hospitalization for patients who are identified as high risk for re-hospitalization. The Patient Retention Specialist also assists the Director of Quality Assurance in obtaining records needed to complete ICD-10 coding.
Qualifications- Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
- Three to five (3-5) years of health care experience, preferred.
- One (1) year experience in home care, preferred.
- Ability to exercise initiative and independent judgment.
- Ability to work with individuals, to enlist cooperation of many people to perform/achieve a common goal.
- Understands and adheres to established Agency policies and procedures.
- Participates in studies, research and other administrative functions as assigned.
- Communicates with high-risk patients at identified time frames.
- Notify Clinical Manager of any signs/symptoms or report of exacerbations for high-risk patients that may warrant a visit.
- Observes confidentiality and safeguards all patient related information.
- Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved.
- Serves as a resource person to employees.
- Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Clinical Services/Clinical Manager.
- Attends pertinent continuing education programs other than routine in services and shares information with staff.
- Communicates with current or discharged patients to ensure that quality services were provided at identified time frames.
- Obtains additional medical records for purposes of ICD-10 coding.
- Upload additional records to the EMR and notify appropriate staff.
- Reviews patient charts in assigned branches requiring Face to Face encounters (FTF). Uploads FTF attachments and processes FTF workflow. Communicates with Care Transition Coordinators and/or physician offices/facilities for needed FTF documentation.

- Ability to participate in physical activity.
- Ability to work for extended period of time while sitting or standing and being involved in physical activity.
- Moderate lifting.
- Ability to do extensive bending, lifting and standing on a regular basis.
