Job Summary
The Registered Nurse (RN) in our Skilled Nursing Facility (SNF) is critical in providing residents with comprehensive nursing care. RNs are responsible for assessing residents' health, developing and implementing care plans, administering medications, coordinating with the healthcare team, and ensuring residents' overall well‑being and comfort.
Key Responsibilities- Nursing Assessment:
Conduct comprehensive assessments of residents' physical and mental health, including gathering medical history, performing physical examinations, and evaluating their current conditions. Identify changes or abnormalities and report them to the appropriate healthcare professionals. - Care Planning:
Develop and implement individualized care plans based on residents' needs, preferences, and medical conditions. Collaborate with interdisciplinary healthcare team members to establish goals, interventions, and approaches for effective care. - Medication Administration:
Administer medications, including oral, topical, and injectable medications, as prescribed. Ensure accurate dosage, monitor residents' medication responses, and promptly address any adverse reactions. Maintain proper medication documentation and assist with medication reconciliation. - Treatment and Procedures:
Perform various treatments and procedures as ordered by physicians, such as wound care, IV therapy, catheter care, and tracheostomy care. Follow infection control protocols to prevent infections and promote healing. - Supervision and Delegation:
Provide guidance and supervision to licensed practical nurses (LPNs), certified nursing assistants (CNAs), and other healthcare personnel. Delegate tasks appropriately and ensure they are performed safely and effectively. - Resident and Family
Education:
Educate residents and their families about their medical conditions, treatment plans, and self‑care techniques. Provide instructions on medication management, disease management, and lifestyle modifications. Address any questions or concerns. - Collaboration and Communication:
Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate and integrate resident care. Communicate effectively with the healthcare team, residents, and families to ensure continuity of care. - Documentation and Reporting:
Maintain accurate and up‑to‑date documentation of assessments, care plans, interventions, and resident responses. Complete necessary forms, charts, and reports per facility policies and regulatory requirements. - Emergency Response:
Respond promptly and effectively to medical emergencies and crisis situations. Follow established protocols for emergency care, including initiating appropriate interventions, providing CPR if necessary, and notifying the appropriate personnel. - Compliance and Quality Assurance:
Adhere to legal and regulatory requirements, including state nursing practice acts, facility policies, and quality assurance initiatives. Stay updated on best practices, evidence‑based guidelines, and professional standards in nursing.
- Graduation from an accredited nursing program and current Registered Nurse (RN) licensure.
- Knowledge of nursing principles, practices, and procedures.
- Strong clinical assessment and critical thinking skills.
- Proficiency in medication administration and knowledge of common medications and their effects.
- Excellent communication and interpersonal skills.
- Ability to work effectively as part of a healthcare team and lead others.
- Attention to detail and accuracy in documentation.
- Compassionate and empathetic demeanor.
- Physical stamina and the ability to perform tasks that may involve lifting, bending, and standing for extended periods.
- Familiarity with electronic health record (EHR) systems and healthcare technology.
- Willingness to work flexible hours, including weekends and holidays, per the facility's needs.
- Works in office area(s), warehouse, pharmacy, and some positions work in a long‑term care environment (e.g., nursing home, drug rooms, nurses’ stations, resident rooms, etc.).
