The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with physician orders, individualized care plans, facility policies, and state and federal regulations. The LPN works closely with the interdisciplinary team to promote the health, safety, dignity, and well-being of every resident.

• Administer medications and treatments as prescribed.
• Assess residents and report changes in condition to the physician and RN Supervisor.
• Document resident care accurately and timely in the electronic medical record.
• Supervise Certified Nursing Assistants (CNAs) and provide guidance as needed.
• Ensure quality nursing care while maintaining resident dignity and respect.
• Respond promptly to resident needs and emergencies.
• Collaborate with physicians, therapists, families, and the interdisciplinary team.
• Follow infection prevention, safety, and regulatory guidelines.
• Participate in care planning and quality improvement initiatives.
• Promote a positive, compassionate, and resident-centered environment.

• Current South Carolina Licensed Practical Nurse (LPN) license in good standing.
• Current CPR certification (or ability to obtain).
• Long-term care or skilled nursing experience preferred, but new graduates are encouraged to apply.
• Excellent communication and organizational skills.
• Ability to work independently and as part of a team.
