Overview
The Licensed Practical Nurse in this position will focus on Annual wellness visits and provide high level patient-centered care in a physician office setting. Duties include measuring and recording vital signs, preparing patients for exams and procedures, assisting the health care provider during exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. The LPN may administer medications, handle patient phone calls per practice protocols, instruct patients regarding health care and treatments, and provide the health care provider with information pertaining to patients’ health status.
This position may be required to float to other UFHP practices as needed for staffing. Travel is involved to coordinate patient wellness visits under the direction of the CHFM Medical Directors, Clinical Faculty, and Clinic Managers.
The Medical Assistant in this position will focus on Annual wellness visits and provides patient care in a physician’s office setting under the direct responsibility of a licensed physician. Clinical skills are within the scope of a Medical Assistant as defined by Florida Statutes 458.3485 and UFHP Clinical Policy and Procedure. The MA may also perform clerical, insurance and medical record functions.
This position also involves travel to coordinate patient wellness visits under the direction of the CHFM Medical Directors, Clinical Faculty and Clinic Managers.

QualificationsLPN: Minimum
Education and Experience Requirements:
Licensed LPN eligible to practice in the State of Florida. One year of ambulatory care or hospital experience preferred. Must display initiative, multitask effectively, and possess outstanding patient/customer service skills, verbal clarity, teamwork, and problem-solving abilities. Licensure/Certification/Registration:
Current Licensed LPN eligible to practice in Florida. CPR/BLS required or must be obtained within first 90 days of employment. Must maintain current CPR/BLS. Must successfully complete a medication test within 30 days of hire.
Licensure/Certification/Registration: Current licensure by the State of Florida as a Licensed Practical Nurse (LPN). CPR/BLS required or must be obtained within first 90 days of employment. Must maintain current CPR/BLS and successfully complete medication test within 30 days of hire.
Medical Assistant: Minimum
Education and Experience Requirements:
Must be a graduate of an accredited course of study as a Certified or Registered Medical Assistant. Certifications include, but are not limited to, AAMA, RMA, NHA, NCCT, AMCA, AAMA and AMT. EMT license may be used in lieu of MA certification. One year of direct patient care experience in an ambulatory setting preferred. Must display initiative, multitask, and be able to assume additional responsibilities.
Must be a team player with strong problem-solving skills.
Licensure/Certification/Registration: Must possess and maintain current certification or registration as a Medical Assistant. CPR/BLS required or must be obtained within first 90 days of employment. CPR/BLS must be maintained. Certification/licensure must be through AHA or Red Cross. Motor Vehicle Operator designation: none required for business purposes.
