Job Description
  • Build trusting relationships with clients and families through regular home visits.
  • Maintain a dependable, punctual schedule with a minimum of 80% completed home visits per level requirement.
  • Assist in strengthening parent/child interaction by providing in‑home education, utilizing established program curriculum, and modeling appropriate behavior.
  • Travel to participants’ homes for regular visits, outreach meetings, and agency collaborations; attend the Healthy Family office for supervision, staff meetings, and training.
  • Establish and periodically update a family support plan, goals, objectives, and activities with supervisor and client within 90 days of participant agreement, reviewed at least every 180 days.
  • Administer Healthy Families America (HFA) tools and Home Safety Checklists at the required intervals, teaching families on topics such as shaken baby, co‑sleeping, water hazards, emergency preparedness, and poison prevention.
  • Document results and outcomes for supervisor review and file them in the client’s HFA chart.
  • Demonstrate knowledge of community resources, make referrals, and follow up to ensure families’ needs are met.
  • Maintain communication with community agencies and providers.
  • Meet with supervisors weekly (minimum 1.5 hours) for case management of all families on caseload.
  • Complete and submit documentation of observations, interactions, and teaching within 72 hours of each home visit; complete all other required forms on time per protocol.
  • Attend regular staff meetings and case conferences as required.
  • Participate in continuing education training and share knowledge with team members.
  • Assist in program growth and development, perform QA audits for families, and perform other duties as needed.
Work Schedule
  • Daily, Monday through Friday, 08:00 AM to 04:30 PM
  • Some flexibility may be required after 05:00 PM and/or on weekends to accommodate home visits.
Compensation

Salary
: $19.15 per hour.

Paid family leave is available for up to eight weeks over 24 months. UF also offers a Gator Perks discount program.

Minimum Requirements

High school diploma or equivalent and one year of experience working with children or families.

  • Ability to establish trusting relationships.
  • Experience working with or providing services to children and families.
  • Acceptance of individual differences.
  • Experience and willingness to work with various populations present among the program’s target population.
  • Knowledge of infant and child development.
  • Open to reflective practice (e.g., capacity for introspection, awareness of self in relation to others, recognition of supervision value).
