Job Description

Healthy Families is a nationally accredited, statewide program that is proven to prevent child abuse and neglect before it ever begins. The program provides community-based home visitation services focused on promoting child health and development and positive parent-child interaction. Services include Parent Education & Support and Mental Health Counseling to participants suffering from Postpartum Depression or other Mental Health Issues.

Responsibilities
  • Establish a trusting relationship with clients and families through regular home visits.
  • Maintain dependable, punctual work schedules with a minimum of 80% completed home visits per level requirement.
  • Assist strengthening parent/child interaction by providing home education, utilizing the established program curriculum, and modeling appropriate behavior.
  • Travel to participant's homes for regular home visits and to community outreach meetings and various agencies for collaboration.
  • Travel to the Healthy Family office for supervision, staff meetings, and training.
  • Establish a family support plan with goals, objectives, and activities within 90 days of participant agreement and review/update at least every 180 days.
  • Administer Healthy Families America (HFA) tools, Home Safety Checklists, and provide associated teachings.
  • Document results for supervisor review and file in client HFA chart within 72 hours after each visit.
  • Make referrals and provide follow-up to ensure community resources meet family needs.
  • Maintain appropriate communication with community agencies and providers.
  • Meet with supervisors weekly (1.5 hours minimum) for case management of all families on caseload.
  • Submit all required documentation on time per written protocol; attend regular staff meetings and case conferences as required.
  • Participate in continuing education training and share knowledge with other team members.
  • Assist and support ongoing program growth and development and perform QA audits for families as directed by supervisor.
Normal Work Schedule
  • Daily, Monday through Friday: 08:00 AM to 04:30 PM
  • Total hours per week: 40
  • Some flexibility may be required to accommodate home visits (after 05:00 PM and/or on weekends).
Salary

$19.15 hourly

Minimum Requirements
  • High school diploma or equivalent and one year of experience working with children or families.
Preferred Qualifications
  • Ability to establish trusting relationships.
  • Experience working with or providing services to children and families.
  • Acceptance of individual differences.
  • Experience and willingness to work with various populations present among the program’s target population.
  • Knowledge of infant and child development.
  • Open to reflective practice (capacity for introspection, communication of self-awareness, and value of supervision).
