Clinical Pharmacist – LTACH

Specialty Medical Staffing is currently seeking a Clinical Pharmacist for a 13-week contract opportunity with a specialty inpatient hospital in the Gainesville, FL area. This position offers a consistent weekday schedule and the opportunity to work closely with an interdisciplinary healthcare team supporting medically complex patients.

We are seeking a pharmacist with strong inpatient clinical experience, excellent communication skills, and the ability to provide direct patient care through medication management, discharge counseling, and clinical pharmacy services.

Assignment Details

  • Start: ASAP
  • Duration: 13 Weeks
  • Schedule: Monday–Friday
  • Hours: 8-Hour Day Shifts, 40 Hours Per Week

Requirements

  • Active Florida Pharmacist License in good standing
  • Recent inpatient hospital pharmacy experience required
  • EPIC experience preferred
  • Strong clinical knowledge of antimicrobial stewardship and pharmacokinetic dosing
  • Experience with medication reconciliation and patient discharge counseling preferred

Responsibilities

  • Perform medication order verification and support daily inpatient pharmacy operations
  • Complete medication reconciliation and discharge medication reviews
  • Provide discharge counseling to patients and caregivers regarding medication therapy
  • Monitor antimicrobial therapy and provide recommendations based on culture and sensitivity results
  • Manage and monitor anticoagulation and pharmacokinetic dosing regimens
  • Review patient medication profiles for drug interactions, allergies, contraindications, and therapeutic appropriateness
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
  • Support formulary management, medication safety initiatives, and quality improvement programs
  • Maintain compliance with state and federal pharmacy regulations and hospital policies
  • Participate in clinical pharmacy services, medication use evaluations, and interdisciplinary patient care activities
