Clinical Pharmacist – LTACH
Specialty Medical Staffing is currently seeking a Clinical Pharmacist for a 13-week contract opportunity with a specialty inpatient hospital in the Gainesville, FL area. This position offers a consistent weekday schedule and the opportunity to work closely with an interdisciplinary healthcare team supporting medically complex patients.
We are seeking a pharmacist with strong inpatient clinical experience, excellent communication skills, and the ability to provide direct patient care through medication management, discharge counseling, and clinical pharmacy services.
Assignment Details
- Start: ASAP
- Duration: 13 Weeks
- Schedule: Monday–Friday
- Hours: 8-Hour Day Shifts, 40 Hours Per Week
Requirements
- Active Florida Pharmacist License in good standing
- Recent inpatient hospital pharmacy experience required
- EPIC experience preferred
- Strong clinical knowledge of antimicrobial stewardship and pharmacokinetic dosing
- Experience with medication reconciliation and patient discharge counseling preferred
Responsibilities
- Perform medication order verification and support daily inpatient pharmacy operations
- Complete medication reconciliation and discharge medication reviews
- Provide discharge counseling to patients and caregivers regarding medication therapy
- Monitor antimicrobial therapy and provide recommendations based on culture and sensitivity results
- Manage and monitor anticoagulation and pharmacokinetic dosing regimens
- Review patient medication profiles for drug interactions, allergies, contraindications, and therapeutic appropriateness
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
- Support formulary management, medication safety initiatives, and quality improvement programs
- Maintain compliance with state and federal pharmacy regulations and hospital policies
- Participate in clinical pharmacy services, medication use evaluations, and interdisciplinary patient care activities
