Physical Therapist Position
Deliver skilled physical therapy services in accordance with the Florida Practice Act and professional standards:
- Perform comprehensive patient evaluations and develop individualized treatment plans
- Establish measurable goals and track functional progress
- Educate patients and caregivers to promote safety and independence
- Supervise and support Physical Therapy Assistants (if assigned)
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary healthcare teams
You will practice with clinical autonomy while being supported by the Director of Rehab and a collaborative homecare team.
Must successfully pass an AHCA Level 2 screening for this position.

Qualifications:
- Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or Doctorate)
- Current Florida Physical Therapist license (in good standing)
- Home health experience preferred
- Must successfully pass AHCA Level 2 background screening
- Frequent driver (daily travel required; mileage reimbursement provided)
- Valid Florida Driver's License
This position requires regular travel to patient homes, defined as:
- Driving daily for business purposes
- Five or more trips per week
- Or averaging over 150 miles per week
