Community Hospice & Palliative Care seeks an APRN to deliver expert palliative and hospice care in diverse settings. The APRN conducts evaluations, prescribes treatments, guides goals of care discussions, and supports transitions of care.
PRN hours M-F with some weekend availability; participates in on-call rotations, staff education, and interdisciplinary team meetings to improve quality of life for patients and families.
