As a CAT Case Manager
, you will provide hands-on, community-based case management services to youth and families:
Meet with youth and families weekly in their home or community setting
Assess needs and develop individualized service plans
Connect clients to resources including healthcare, education, housing, and employment
Coordinate care with families, providers, and community partners
Advocate for clients and support long-term stability and independence
Assist with transportation to appointments, court, or services when needed
Monitor progress and adjust plans to support successful outcomes
Complete documentation in a timely and compliant manner
Majority of work is in the field (homes, schools, community settings)
Driving is required throughout your assigned service areas, Alachua, Baker, Bradford, Gilchrist, Levy, and Union counties
Work may include evenings, weekends, and on-call rotation
You may support youth and families experiencing behavioral, emotional, or crisis situations
Bachelor’s degree in:
Psychology
Social Work
Mental Health Counseling
Marriage & Family Therapy
Or related field
Minimum 1 year of experience working with youth or behavioral health populations
Valid Florida driver’s license, reliable vehicle, and acceptable driving record
Ability to pass Level 2 background screening
Strong communication, organization, and problem-solving skills
Bilingual (Spanish or Creole) a plus
Maintain compliance with Medicaid, DCF, and agency standards
