This role focuses on delivering exceptional care within a Rehab Unit, where you will apply your skills in patient evaluations, ADLs, transfers, and functional training. Key Responsibilities of the Physical Therapist:

• Perform patient evaluations and develop individualized therapy plans
• Conduct ADLs, transfers, and functional training
• Provide family and patient education
• Engage in gait training and therapeutic exercises
• Document care in Meditech (Expanse) system

Requirements of the Physical Therapist:

• Valid BLS certification
• Previous SNF experience is advantageous
