This role focuses on delivering exceptional care within a Rehab Unit, where you will apply your skills in patient evaluations, ADLs, transfers, and functional training. Key Responsibilities of the Physical Therapist:
- Perform patient evaluations and develop individualized therapy plans
- Conduct ADLs, transfers, and functional training
- Provide family and patient education
- Engage in gait training and therapeutic exercises
- Document care in Meditech (Expanse) system
Requirements of the Physical Therapist:
- Valid BLS certification
- Previous SNF experience is advantageous
