Pay Range:$2,600 - $2,800
Schedule: 08:00:00 to 17:00:00 (on-call as needed)
Contract Length: 13 Weeks
Candidate Type: Traveler
Requirements: 1 year of experience as a PT Qualified applicants MUST have at least 1 year of experience in Home Health Valid Physical Therapist license Be willing to obtain Florida or compact licensure

• Deliver one-on-one care to patients in their homes.
• Conduct patient assessments and document findings to support care planning.
• Develop and manage individualized care plans based on patient needs.
• Coordinate care with families and interdisciplinary providers to support patient outcomes.
• Maintain clear, timely communication and follow established procedures during the assignment.
