working title: ops registered nurse -

this ops registered nurse serves as the hug - me nurse for area 15 (st. Lucie, martin, okeechobee, and indian river counties), providing family-centered care coordination to pregnant and postpartum women living with hiv and hiv-exposed or hiv-positive infants.

working under the community health nursing supervisor, the ops hug- me nurse serves clients in homes, hospitals, clinics, provider offices, and community settings. Independent travel and use of a personal vehicle may be required. All fdoh confidentiality and information-security requirements apply.

care coordination and case management

assesses client and family needs and develops, implements, monitors, and evaluates individualized care coordination plans.

reviews clinical records and laboratory results and ensures timely follow-up, referrals, and linkage to medical and supportive services.

collaborate with healthy start, topwa, ryan white, hiv surveillance, dis, hospitals, medical providers, laboratories, pharmacies, and community partners.

provides nursing consultation, education, crisis support, home and hospital visits, appointment assistance, case staffing, and other services needed for continuity and retention in care.

maternal and infant hiv services

educates women about hiv testing, treatment, viral suppression, prevention of perinatal transmission, medication adherence, prenatal and postpartum care, and family-planning services.

meets with mothers before hospital discharge when possible and links mothers and infants to specialty care, medication, laboratory services, transportation, and other supports.

coordinates care for hiv-exposed or hiv-positive infants through 18 months of age, including discharge medication, adherence, diagnostic testing, appointments, and specialty follow-up.

teaches and supports parents or guardians in safely caring for the infant and participating in ongoing care coordination.

provider and program support

educates obstetric, pediatric, hospital, public health, and community providers on current perinatal hiv guidance and serves as an area 15 nursing resource

co-chairs monthly mama bear meetings or calls and participates in required meetings, trainings, client staffing, and outreach events.

documentation and reporting

documents services at the time of care, maintain required client files, and enters accurate and confidential data into hms, careware, ears, and program tracking systems.

completes required weekly, monthly, quarterly, ryan white, hrsa, and annual rsr reports and maintains mama bear and baby bear spreadsheets.

administrative and emergency responsibilities

maintains professional communication, current clinical and program knowledge, and confidentiality under dohp 50-10-10 and assigned security level 3 requirements.

performs assigned emergency, shelter, disaster-preparedness, outreach, and other related duties, including work outside normal hours when required.

required knowledge, skills, and abilities:

knowledge of prenatal, intrapartum, postpartum, interconception, and hiv-exposed infant care in accordance with current hhs/nih perinatal hiv clinical guidelines and applicable florida department of health policies and procedures.

knowledge of public-health nursing principles, family-centered care coordination, case management, confidentiality requirements, and community resources.

ability to review clinical information, establish priorities, coordinate multidisciplinary services, educate clients and providers, document accurately, and work independently in community settings.

computer proficiency sufficient to use hms, careware, spreadsheets, electronic records, communication platforms, and other required systems.

qualifications:

minimum;

  • current, valid florida registered nurse license in accordance with chapter 464, florida statutes.
  • current cpr certification.
  • valid florida driver's license
  • case management experience
preferred:

one year department of health experience

one year of maternal health experience
