As a CAT Team CAT Behavioral Health Case Manager, you will provide hands-on, community-based case management services to youth and families:
- Meet with youth and families weekly in their home or community setting
- CAT Team Behavioral Health Case Managers work a unified group of providers, supporting each other in reaching client goals.
- CAT Team Behavioral Health Case Managers collaborate closely with Therapists, Nurses, Mentors and Psychiatrists.
- Assess needs and develop individualized service plans
- Connect clients to resources including healthcare, education, housing, and employment
- Coordinate care with families, providers, and community partners
- Advocate for clients and support long-term stability and independence
- Assist with transportation to appointments, court, or services when needed
- Monitor progress and adjust plans to support successful outcomes
- Complete documentation in a timely and compliant manner
- Majority of work is in the field (homes, schools, community settings) with flexibility to complete administrative tasks remotely.
- Driving is required throughout your assigned service areas, Alachua and surrounding communities.
- Enjoy flexibility in building your schedule while meeting clients when services are most accessible to them. Depending on client needs, this may include some evening and weekend availability.
- Limited on-call expectations, providing greater consistency and flexibility in your schedule.
- You may support youth and families experiencing behavioral, emotional, or crisis situations.
- Bachelor’s degree:
- Psychology
- Social Work
- Mental Health Counseling
- Marriage & Family Therapy
- Or related field
- Minimum 1 year of experience working with youth with behavioral health populations.
- Bachelor's degree in other fields requires (3) years minimal experience working with youth with behavioral health populations.
- Valid Florida driver’s license, reliable vehicle, and acceptable driving record
- Ability to pass Level 2 background screening
- Strong communication, organization, and problem-solving skills
