HOO M-F 08:30-19:30 (1 hr lunch) Sat 09:00-17:30 Sun 09:00-16:30 (30 min lunch)

• Must be willing to float to cover Gainesville and Mac Clenny (2-3 weekdays and 6/12 weekend shifts).
• Approx. 30-38 hours a week.
• Possible holiday rotations.

• Minute Clinic Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidence-based care to a panel of patients.
• The clinical care team will work with the provider daily to drive better outcomes for the entire panel of patients.
• Tasks include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
• This role reports to the practice manager.

• Provide primary and family evidence-based care, including accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
• Perform administrative duties such as opening/closing the clinic, inventory management, patient follow up, insurance verification, payment collection, and maintain a clean environment.
• Handle patient appointments and perform routine examinations to ensure positive patient health outcomes.
• Conduct health education, diagnostic tests, and coordinate with other healthcare professionals to develop comprehensive care plans.
• Encourage patients to become active participants in their own healthcare and well‑being through services such as health screenings, behavioral health and wellness, and chronic condition management.

• Minimum of 1 year of recent primary care experience as a Nurse Practitioner in family medicine, primary care practice, or urgent care.
• Working knowledge of quality measurement in chronic disease management.
• Self‑motivated, prioritizes and solves problems, takes initiative, and advocates for patients and practice.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi‑task.
• Problem‑solving ability, adaptability, and flexibility.
• Capability to work remotely in a clinical team culture.
• Ability to work without direct supervision and practice autonomously.
• Proficiency with information management and technology.
• Willingness to obtain multi‑state licensures.

• Master's degree level Family Nurse Practitioner program with current National Board Certification and active state licensure to practice as an Advanced Practice Nurse.
