Hospice RN Case Manager (Registered Nurse) – Full Time
Arc Hospice & Palliative Care invites you to join our mission-driven team, where you will bring comfort, dignity, and compassionate support to patients and their families.
Monday–Friday schedule + rotating on‑call. Provide meaningful care and support while delivering skilled, personalized nursing aligned with each patient’s goals and needs.
About the RoleThe Hospice RN Case Manager is responsible for comprehensive nursing assessment, care planning, and ongoing clinical oversight for hospice patients and their families, ensuring high‑quality, compassionate care while coordinating closely with the interdisciplinary team.
Key Responsibilities- Complete initial and ongoing assessments of patients’ physical, emotional, psychosocial, and environmental needs.
- Coordinate with physicians for certification and recertification of terminal illness per Medicare regulations.
- Serve as the clinical point person for assigned patients, including symptom escalation, advanced care planning, and advocacy.
- Collaborate with all providers and support systems to ensure seamless, compassionate hospice care.
- Develop and update individualized care plans with patients, families, and the interdisciplinary team.
- Provide skilled nursing care and document interventions and outcomes accurately and promptly.
- Ensure documentation meets regulatory and billing compliance standards.
- Educate patients and families on disease progression, symptom management, medications, and hospice philosophy.
- Supervise hospice aides and revise their care plans as needed.
- Participate in interdisciplinary group meetings with physicians, social workers, chaplains, and bereavement staff.
- Communicate effectively with the interdisciplinary team and clinical leadership.
- Participate in the on‑call rotation and respond to urgent needs.
- Escalate care needs to the medical director or covering provider when appropriate.
- Perform other duties as assigned.
- Graduate of an accredited nursing program; associate degree or higher preferred.
- Current RN license in the state of practice or a valid multistate compact license.
- Minimum one (1) year of hospice, palliative care, or home health experience preferred.
- CPR certification and a valid driver’s license required.
- Strong clinical judgment, documentation accuracy, and communication skills.
- Knowledge of hospice regulations and Medicare Conditions of Participation preferred.
- Ability to work independently while collaborating across interdisciplinary teams.
- Excellent communication, critical thinking, and organizational skills.

$80,000 – $90,000 USD
Compensation is determined by experience, licensure, location, and business needs at the time of hire. Some roles may be eligible for additional components consistent with internal practices.
