The RN Case Manager is responsible for planning, managing, and coordinating care for an assigned caseload of patients receiving supportive care services. This primarily telephonic role ensures comprehensive, compassionate care through collaboration with an interdisciplinary team. Responsibilities include conducting assessments, care planning, symptom management, patient and caregiver education, and coordination with medical providers and support services

: $68,000 - $72,000 base salary, plus driving paid mileage; full benefits package including 23 days PTO annual
- Conduct initial and ongoing assessments of patients’ physical, functional, psychosocial, and environmental needs.
- Coordinate and implement individualized plans of care aligned with patient and caregiver goals.
- Facilitate goals of care discussions, including advance care planning.
- Provide telephonic support, guidance, and education to patients and caregivers.
- Collaborate with interdisciplinary team members, including medical providers and social workers.
- Monitor patient conditions and communicate symptom management needs.
- Ensure timely and accurate documentation in accordance with organizational standards.
- Participate in interdisciplinary group (IDG) meetings and care planning reviews.
- Comply with applicable local, state, and federal regulations.
- Maintain professional relationships with facilities and community partners.
- Participate in professional development and mentor new staff as needed.
- Perform other related duties as assigned.
- Minimum of three (3) years of experience as a Registered Nurse
- One (1) year of hospice or palliative care experience preferred
- Registered Nurse license in the state of Florida, up-to-date
- Current CPR certification
- Valid Florida driver’s license with an acceptable driving record
- Associate Degree or Bachelor's Degree in Nursing from an accredited nursing program
