Registered Nurse Case Manager - Hospice

Job Overview

We are seeking a dedicated Hospice RN to join our team in Gainesville, FL. This full‑time position runs Monday through Friday, 8a-5p, with no weekends or holidays. Coverage includes Alachua and Bradford Counties, primarily Newberry and High Springs.

Gainesville, FL (Coverage area: Alachua, Bradford Counties)

• Provide 1:1 patient and family–centered care in hospice settings.
• Assess, monitor, and document physical, emotional, and psychological needs.
• Create and implement hospice care plans aligned with patient goals.
• Administer medications, treatments, and interventions.
• Manage pain and other symptoms.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team members.
• Maintain accurate, timely documentation.
• Participate in on‑call rotation as required.

• Graduate of an accredited nursing school with current licensure to practice in Florida.
• Minimum one year nursing experience; three or more years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an insured automobile in good working order.
