Home Health Nurse / Hospice RN (Registered Nurse) - Setting:
Home Health & Hospice – in-home patient care - Contract Duration: 13 weeks (May 4, 2026 – Aug 2, 2026)
- Shift: Monday – Friday, 8:00 AM – 5:00 PM (8-hour shifts)
- Hours per Week: 40
- On-Call: 1 weekend/month plus occasional weekday after-hours call
- Travel:

- Active Registered Nurse (RN) license in Georgia – required
- Minimum 1 year nursing experience; home health or acute care experience preferred
- Experience with OASIS assessment, wound care, negative pressure therapy (NPWT), PICC and central line management, foley insertion/management, PleurX drain management, ostomy care, tracheostomy (trach) care, and LVAD management preferred
- Comfortable managing a caseload independently across multiple counties (Jackson, Barrow, surrounding areas)
- Strong communication, clinical assessment, and EHR/documentation skills
- Perform comprehensive in-home nursing assessments and develop individualized care plans for patients receiving home health and hospice services
- Deliver disease management, health maintenance, symptom management, and palliative care interventions as a Home Health Nurse / Hospice RN
- Observe, assess, plan, implement, and evaluate nursing care for well, chronic, or acutely ill patients
- Coordinate care and communicate effectively with physicians, interdisciplinary team members, caregivers, and community resources
- Manage medication administration, safe storage, reconciliation, and documentation according to policy
- Report changes in patient condition to physicians and facilitate timely order transcriptions and follow-up
- Complete required nursing documentation (daily, OASIS, monthly, quarterly, annual) and maintain accurate EHR records
- Maintain infection control standards for equipment and supplies in the home setting
- Provide precepting, staff education, and participate in new employee orientation and annual competency recertification
- Coordinate scheduling and follow-up for appointments, therapies, and hospital discharges
- Participate in the on-call rotation as assigned to support patient needs across the service area
