Community Hospice & Palliative Care is seeking an RN Case Manager to plan, manage, and coordinate care for a caseload in a telephonic home-care setting around Gainesville. Base salary $68,000–$72,000 with mileage reimbursement and a strong benefits package including 23 days PTO.
The role requires 3+ years as an RN, FL RN license, CPR, and a valid FL driver's license; a nursing degree such as ADN or BSN is required. You will work with an interdisciplinary team to deliver patient-centered care.
