Overview
The North Florida/South Georgia VA Healthcare System's Social Work Service is seeking an experienced and dynamic Registered Nurse Outpatient Caregiver Support Program Clinical Assessor. Preferred experience includes 1-2 years of case management in a primary care/mental health setting.
Qualifications- English Language Proficiency
- In accordance with 38 U.S.C. 7403(f) - no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following: ACEN or CCNE
- OR Individuals attending a master s level bridge program in nursing who have completed coursework equivalent to a bachelor s level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program
- Upon achievement of a State license - the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program

- OR For graduates of foreign schools of professional nursing - possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing
- Credit for foreign nursing education higher than an associate degree/entry level requires formal degree equivalency validation from a recognized evaluation service
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (e.g., Puerto Rico) or the District of Columbia
- Graduate Nurse Technician (GNT) Exception:
Candidates who meet basic education requirements but do not possess licensure may be appointed at the entry step of the grade and level on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case basis).

- Physical Requirements:
Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
- The Caregiver Support Program Clinical Assessor Nurse is responsible for knowledge of eligibility criteria for the PCAFC (Program of Comprehensive Assistance for Family Caregivers) and day-to-day operation of the program within the Veterans Integrated Systems Network
- Delivers knowledge-based care and counseling to assigned clients while developing technical skills
- Executes position responsibilities that demonstrate beginning leadership and participates in PCAFC experiences
- Demonstrates creative approaches to managing aspects of complex patient care
- Provides care and counseling aimed at health promotion and illness/injury prevention based on the nursing process
- Maintains thorough knowledge of PCAFC – clinical documentation, current standards of care, and regulatory requirements
- Uses critical thinking and sound judgment to ensure caregivers receive thorough, comprehensive care options within CSP scope
- Assess, plan, implement, and evaluate care with a focus on age-appropriate components; coordinate care with emphasis on education, self-management, and satisfaction
- Influences care outcomes by collaborating with interdisciplinary team; accountable for all elements of the nursing process in direct patient care
- Demonstrates leadership, experience, and creative approaches to management of complex patient care; applies CSP and CEAT practice; evaluates current practice and formulates outcomes for groups or processes
