Job Summary

Job Responsibilities- Initiate, develop, and implement intermediate nursing plans of care, evaluate patient progress, and modify treatment plans in response to changing status or physician orders.
- Assess patient needs and physical status during each skilled visit, re-evaluate through reassessment and supplemental orders, and obtain clarification from physicians as necessary.
- Provide training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
- Teach patients and families about a patient‑centered care approach to achieve independence in therapy and care.
- Communicate effectively with the clinical team, medical providers, patients, and families to facilitate continuity of care; maintain real‑time documentation and submit to nursing department within billing timelines.
- Coordinate discharge planning, prepare discharge summaries, and report patient progress when transferring care.
- Ensure compliance with agency operations, policies, professional standards, regulatory guidelines, and accreditation requirements.
- Maximize work efficiency through use of computers and automation technologies to validate orders and follow visit schedules.
- Respond promptly to patient requests and initiate emergency procedures as required.
- Maintain ethical and professional conduct; adhere to established programs and policies that reflect Option Care’s mission and values.
- Safeguard patient confidentiality and proprietary company information in accordance with HIPAA regulations.
- Promote knowledge of Option Care’s services and explain related financial responsibilities to patients.
- Complete required training, in‑services, and continuing education; maintain competency and professional licensure for care and management of patients in home or alternate settings.
- Provide oversight and coordination of paraprofessionals and may act as supervisor in the absence of the Nurse Manager, per state and federal regulations.
- Participate in multidisciplinary team conferences, precepting, orientation, onboarding, and supervisory activities as assigned.
- Participate in nursing department on‑call responsibilities.
- Perform other related duties as directed by supervisor.
- Insert PICC/Midlines per physician order.
- Troubleshoot or declot obstructed central line catheters.
- Monitor PICC/Midline and educate on prevention of line infection and routine care.
No supervisory responsibilities for this position.
Basic Education And/Or Experience- Active and unrestricted RN license in the state of practice.
- Minimum of 1 year of nursing experience.
- Current CPR certification required.
- Demonstrated competency in patient care standards and infusion skill sets applicable to agency programs.
- Required licensure to operate a motor vehicle in the state of practice with a reliable vehicle and proof of liability insurance.
- Basic computer skills and ability to learn and apply nursing technology tools for communication and documentation.
- Lift up to 50 pounds (up to 75 pounds occasionally); perform up to 20 pounds of force occasionally or 10 pounds frequently; duties include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching, grasping, pushing, pulling, lifting and carrying.
- Exposure to blood, body fluids, and other potentially infectious materials; potential exposure to hazardous substances and unpredictable home environments.
- Wear closed‑toe shoes, facemask, goggles, gown, gloves, and a functioning communication device in routine and emergency situations.
- Willing to travel up to 100% of the time for business purposes.
- Bachelor of Science in Nursing from an accredited school of nursing preferred.
- 1–2 years of previous infusion nursing experience preferred.
Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Pay Range is $36.89–$61.49.
