Nursing LPN​/LVN

Sevita is seeking a compassionate LPN/LVN to deliver person-centered care in home and community-based settings. In this role, you'll provide direct nursing care, administer medications and treatments, monitor health status, and accurately document care in alignment with individualized care plans. You'll collaborate closely with an interdisciplinary team, support individuals with diverse medical and behavioral needs, and help them live as independently as possible.

Responsibilities

• Provide direct patient care and monitoring in home and community-based settings
• Administer medications and treatments per physician orders and nursing protocols
• Document patient status, interventions, and outcomes accurately and timely
• Support individualized care plans focused on independence and quality of life
• Collaborate with RNs, physicians, therapists, and caregivers to coordinate care
• Educate individuals and families on health conditions, medications, and self-care
• Respond to changes in patient condition and report concerns promptly
• Follow infection control, safety, and regulatory standards at all times
• Participate in training, supervision, and professional development activities
