The Community Case Manager offers services in a housing setting, integrating evidence-based practices with individuals who have experienced chronic homelessness and have mental and/or physical health needs. Such services include outreach, conducting assessments, developing service plans, providing intensive case management, increasing access to benefits/healthcare/mental healthcare and community integration. RRH/PSH Case Manager will implement evidence-based practices such as Housing First, Harm Reduction, Motivational Interviewing and Trauma-Informed Care.
Case Manager will work collaboratively with a range of community service providers to ensure participants are stabilizing in housing, improving the quality of their lives, and integrating into the community.
- Create a person centered goal plan in collaboration with the participant addressing short and long term goals.
- Utilize HMIS for all documentation.
- Provide inhome case management services in the areas of, but not limited to: independent living skills, housing stabilization, money management, community integration, employment, linkage to community providers for problematic substance use, primary and mental health care, and all other services needed to assist participants in reaching their goals.
- Meet with Participants at minimum twice monthly to provide inhome case management.
- Use evidence based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, and Housing First practices.
- Provide inhome case management services in areas of independent living skills, community integration, employment, benefits establishments, and other services needed to assist participants in reaching case plan goals.
- Mediate and advocate on behalf of the participant to help him/her obtain and keep housing, health care benefits and entitlements.
- Pass a level 2 background check.
- High School Diploma or equivalent
- 2 year’ social service experience. Preferred in homeless services or case management.
- Self directed and able to work without much supervision.
- Superior organizational and communication skills (written & oral)
- Ability to interact with all persons in a professional manner encompassing behavior, dress, language, and attitude.
- Basic computer skills: proficiency with GSuite, HMIS and Slack.
- A valid driver’s license and insurance and reliable transportation.
This role has a number of physical demands of staff. GRACE has approximately 30 scattered site apartments. You will be driving from site to site the majority of your day. Participants’ apartments are located on the first and second floors so you will need to be able to walk up and down stairs. This role is to ensure that people do not lose their housing which will sometimes require assisting with move-ins and outs so you will occasionally need to lift up to 50 lbs.
Case Managers will also assist with cleaning participants’ apartments as needed.
