Job Description

Establishes a trusting relationship with the client and family by maintaining regular home visits as required per client level. Maintains dependable, punctual work schedules with a minimum of 80% completed home visits per level requirement. Assists in strengthening parent/child interaction by providing in‑home education, utilizing established program curriculum and modeling appropriate behavior during each home visit. Travels to participant's homes for regular visits and to creative outreach meetings and various agencies for collaboration.

Works from the Healthy Family office for supervision, staff meetings and training. Establishes a family support plan with goals, objectives, and activities with supervisor and client within 90 days of participant agreement. This plan directs every home visit and is reviewed and updated at least every 180 days. Administers Healthy Families America (HFA) tools at the required intervals for each family.

Administers Home Safety Checklists with all families, per schedule, and provides associated teachings as required, including Shaken Baby, co‑sleeping, water hazards, emergency preparedness, and poison prevention. Files documentation in client's HFA chart. Demonstrates excellent knowledge of community resources, makes referrals, and follows up to ensure needs are met. Maintains communication with community agencies and providers. Meets with supervisors weekly (1.5 hours minimum) for case management of all families on caseload.

Documents observations, interactions, and teaching at each visit and submits to supervisors within 72 hours. Attends regular staff meetings and case conferences as required. Participates in continuing education training and shares knowledge with other team members. Supports ongoing program growth and development. Performs QA audits for families as directed. Performs other duties as required to meet program objectives.

Normal Work Schedule

Daily, Monday through Friday, 8:00 AM to 4:30 PM. Some flexibility may be required to accommodate home visits after 5:00 PM and/or on weekends.

Expected Salary

$19.16 hourly.

Required Qualifications

High school diploma or equivalent and one year of experience working with children or families.
