What You'll DoAs a Discharge Planner, you will play a key role in supporting patient care and recovery by:
- Establishing and maintaining a welcoming, therapeutic relationship with clients
- Collaborating with physicians, nurses, emergency screeners, counselors, and other staff to ensure high‑quality patient care
- Coordinating discharge planning and follow‑up care to reduce readmissions
- Conducting family meetings and gathering collateral information to support treatment and discharge plans
- Actively participating in daily treatment team meetings to assess client progress and outcomes
- Leading psycho‑educational groups and therapeutic activities as directed
- Assisting in client safety interventions, including participation in restraints when necessary
- Supporting program and departmental goals through additional duties as assigned
Minimum Qualifications
- Bachelor's degree in social work, Counseling, Psychology, or a related field
- New graduates are encouraged to apply
- Prior experience in discharge planning or case management preferred (especially in behavioral healthcare settings)
