The LTC Admissions/MDS Coordinator functions as the Admissions Coordinator for the facility or a portion of the facility as assigned. This person assists in coordinating the assessment of potential LTC residents, works with potential residents and their families in planning for LTC placement, conducts admission conferences, assists and follows up on financial assistance applications, and is responsible for the timely and accurate completion of assigned MDS and Care Plans to enhance quality resident care.
The position will care for patients in the adult and geriatric age groups.
- Coordinate assessments of potential LTC residents and assist in admission conferences.
- Work with potential residents and families to plan LTC placement and conduct facility visits.
- Maintain and complete timely MDS and care plans, ensuring quality of care.
- Assist with financial assistance applications (Medicaid, Disability, etc.) and follow up on approvals.
- Act as resident’s advocate, communicating changes in their condition to the care team.
- Facilitate the admission of residents, including scheduling admission conferences, reviewing paperwork, and managing financial obligations.
- Coordinate admission, transfer, and discharge processes and refer problems to the Director of Nursing when necessary.
- Provide resident teaching and continuing education for residents, families, and staff.
- Gather data, interview stakeholders, and facilitate disposition decisions.
- Monitor resident physical and mental status and respond to changes in their condition.
- Licensure:
Current LPN or RN licensure with the Georgia Board of Nursing. - Education:
High School Diploma or GED. - Experience:
Minimum two (2) years of experience in long‑term care; prior management experience is a plus.
- Preferred Licensure: LPN or RN.
- Preferred Education:
Any additional certification or training related to long‑term care. - Preferred Experience:
Prior experience as an Admissions Coordinator or similar role. - Knowledge, Skills, and Abilities:
Good clinical and computer skills, strong organizational and leadership abilities, positive interpersonal skills, ability to interview and gather data, and to facilitate disposition.
