Kidney Care Center of Georgia – Northeast Georgia’s Premier Nephrology group is expanding. We are looking for a motivated Advanced Practice Provider (Acute Care NP or PA) to join our clinical practice. The provider will give patient-centered, high-quality care to patients with kidney disease and related conditions. The APP will assist clinicians in managing acute kidney injury, chronic kidney disease, and end-stage renal disease patients.
Responsibilities- Provide inpatient care, including in the intensive care units.
- Conduct initial evaluation and management of patients.
- Follow up on patient status and provide dialysis services (hemodialysis).
- Offer office care for ongoing treatment of CKD and post-hospital follow-up.
- Coordinate in-center dialysis visits for End Stage Kidney Disease patients.
- Document and bill appropriately according to requirements.
- Coordinate plan of care, discharge, and specialty referrals.
- Plan and coordinate health-care education for patients, families, and other providers.
- Assist with procedures per collaborative agreement.
- Participate in performance improvement activities.
- Master's degree in Nursing with an Acute Care NP certification, or graduate from an accredited Physician Assistant program.
- Currently certified as an Advanced Practice Nurse (APN) with Certified Registered Nurse Practitioner (CRNP) specialty by the Georgia Board of Nursing, or licensed as a Physician Assistant by the Georgia Board of Medical Examiners with certification as a Physician Assistant (PA-C) by the National Commission on Certification of Physician Assistants.
- Licenses/registrations must be active; education cannot be substituted for required training.
Monday‑Friday, 8:00 am‑5:00 pm. Weekend coverage required maximum once per month.
