Licensed Practical Nurse (LPN) – Home Care Supervisory Visits

A&C Private Home Care is seeking a compassionate, dependable, and detail-oriented Licensed Practical Nurse (LPN) to join our team. This position is responsible for conducting supervisory visits, completing nursing assessments, ensuring quality client care, and maintaining compliance with state regulations, Medicaid waiver programs, and Veterans Affairs (VA) requirements.
The ideal candidate is organized, professional, and committed to promoting quality care while supporting clients, caregivers, and the agency's mission.
Responsibilities- Conduct routine supervisory visits in clients' homes to assess the quality of care being provided.
- Complete nursing assessments, supervisory visit documentation, and all required clinical paperwork accurately and timely.
- Monitor clients' health status and report significant changes to the appropriate healthcare provider, physician, case manager, or agency leadership.
- Evaluate caregiver performance and provide education, coaching, and recommendations to promote quality care.
- Ensure caregivers are following each client's care plan and agency policies.
- Identify safety concerns within the home and recommend appropriate interventions.
- Maintain compliance with state regulations, Medicaid waiver programs, Veterans Affairs (VA) requirements, and agency policies.
- Collaborate with Care Coordinators, management, physicians, hospitals, and case managers to ensure continuity of care.
- Participate in quality assurance and performance improvement activities.
- Maintain client confidentiality and comply with all HIPAA regulations.
- Complete all documentation and supervisory visits within required time frames.
- Perform other nursing duties as assigned.
- Current, unrestricted Licensed Practical Nurse (LPN) license.
- Current CPR certification.
- Valid driver's license, reliable transportation, and proof of automobile insurance.
- Ability to travel throughout the assigned service area.
- Strong assessment, documentation, and communication skills.
- Excellent organizational and time management abilities.
- Ability to work independently while maintaining professionalism.
- Experience in home care, hospice, long-term care, or community health is preferred.
- Knowledge of state home care regulations, Medicaid waiver programs, Veterans Affairs (VA) requirements, and regulatory compliance is highly preferred.
- Strong clinical assessment skills.
- Excellent documentation and electronic charting skills.
- Ability to recognize changes in client condition and communicate effectively.
- Knowledge of infection control, client safety, and quality assurance practices.
- Strong interpersonal skills with the ability to build positive relationships with clients, families, caregivers, and referral sources.
- Ability to prioritize multiple supervisory visits while meeting required deadlines.
