Care Manager – Marketing Coordinator

Responsibilities

• Coordinate caregiver schedules and client assignments
• Assist with onboarding new caregivers
• Maintain caregiver and client records
• Follow up on missed visits and scheduling changes
• Coordinate nursing assessments and client admissions
• Assist with care plans and documentation
• Monitor Well Sky documentation and compliance
• Communicate with caregivers, nurses, clients, and families
• Answer phones and respond to inquiries
• Maintain confidentiality and HIPAA compliance
• Build relationships with hospitals, rehabilitation centres, assisted living communities, physicians, and senior centres
• Conduct community marketing visits
• Deliver brochures and promotional materials
• Attend networking events, health fairs, and community events
• Develop referral relationships
• Track referral sources and marketing efforts
• Assist with recruitment initiatives

• High School Diploma or GED required
• Associate's or Bachelor's degree in Healthcare Administration, Business, Marketing, or related field preferred
• Previous healthcare, home care, medical office, or customer service experience preferred
• Strong computer skills (Microsoft Office, Outlook)
• Excellent communication and organizational skills
• Ability to multitask in a fast-paced environment
• Professional appearance and demeanor
• Valid driver's license and reliable transportation.

Demonstrates strong organizational and communication skills while coordinating caregiver schedules, client assignments, and maintaining compliance with HIPAA regulations. Proficient in building relationships within the healthcare community and assisting with recruitment and marketing initiatives.
