Job Description

Licensed Practical Nurse (LPN) position providing day‑to‑day support for medication administration program and ensuring quality resident care under the direction of the Executive Director and Delegating RN.

Educational / Licensing Requirements
  • LPN or higher
  • Graduate of an accredited school of registered or licensed practical nursing
  • Licensed in good standing in Maryland or a Nurse Licensure Compact state
  • Active unencumbered license to practice as an RN or LPN
  • CPR certification or to obtain within 60 days of hire
Responsibilities
  • Coordinate with the Community Manager and Resident Service Coordinator for resident service assignments and schedules based on optional services and assessments.
  • Recognize when a resident requires additional help and coordinate services with the Community Manager, Resident Services Coordinator, family members, outside community services, and physicians.
  • Support Delegating Nurse oversight of the medication administration program, including:
    • Review all physician orders.
    • Transmit new orders to pharmacy or treatment provider and place them on the MAR.
    • Ensure proper paperwork accompanies residents to physician appointments and review returned paperwork.
    • Take appropriate actions in response to changes or new orders.
    • Accurately administer ordered medications and treatments.
    • Maintain accurate MAR charting and coach staff on corrections.
    • Ensure timely and accurate medication filling and verify CMT monthly medication cycle from pharmacy.
    • Report any changes in resident’s medication reaction to physician and facility staff.
  • Notify Delegating Nurse, Community Manager, family or responsible party, and physician of any significant changes in the resident’s status.
  • Document weekly resident notes and review CMT weekly notes regarding residents.
  • Monitor status of residents out at hospitals and rehabs, ensuring safe return to the facility before discharge. This includes:
    • Obtain and review resident medical reports and discharge orders.
    • Consult with family and physicians during discharge planning.
    • For medication administration program residents, ensure all medication orders are signed and sent to pharmacy to have medications on‑hand upon discharge.
  • Maintain constant alertness to help residents maintain or improve functional abilities within present capabilities.
  • Observe and record pertinent symptoms, profile, and progress of residents.
  • Comply with facility policies, procedures, forms, and documentation.
  • Remain aware of problems of elderly and disabled persons; avoid unnecessary noise, obstructing machinery, and ensuring compliance with smoking restrictions.
  • Maintain professional conduct reflecting the profession and facility’s philosophy.
  • Participate in hiring, performance evaluations, terminations, and progressive discipline of direct care staff.
  • Monitor CMT performance of health‑related activities; identify gaps and provide staff development and training.
  • Observe and protect the rights of all residents, including confidentiality, privacy, and dignity.
  • Practice safety procedures and report accidents and injuries.
  • Perform any other related duties assigned.
  • Schedule: Part Time LPN weekdays; flexible hours.
