Sheppard Pratt is searching for a Registered Nurse to serve in a non-supervisory role as Assistant Director for its Health Home program. The position will split time between Gaithersburg and Lanham, MD.
Sheeppard Pratt's Behavioral Health Home supports clients who are receiving behavioral health services through various Sheppard Pratt programs and need additional physical healthcare support as part of their recovery journey. What to ExpectIn this role, you will treat the "whole person" by identifying gaps and barriers between behavioral and physical healthcare to develop prevention initiatives that ensure all care needs are met.
Once assigned a patient, you'll ensure the implementation of their treatment plan in partnership with program leadership and staff, the individual, and their support system. You will be asked to:
- Monitor treatment plan goals.
- Coordinate access to medical providers and hospitals, including admission and discharge.
- Ensure patients complete all required assessments, screenings, and immunizations.
- Keep abreast of current trends and developments in the field.
- Participate in the evaluation of program effectiveness and ensure continuous improvements in the quality of services.
- Assist in the occasional training of Health Home staff, specifically nurses.
- Identify quality improvement opportunities and population-level care management.
- Develop and maintain partnership with external entities (hospitals, medical providers, and community organizations) that play a role in treatment plans.
The ideal candidate brings strong interpersonal skills, experience in care coordination or case management, and a genuine ability to build trust and meaningful connections with individuals experiencing mental illness.
Requirements- Must be a Registered Nurse in Maryland at time of application.
- Must have 1-3 years of nursing experience.
- Must have car insurance and access to a vehicle.
The pay range for this position is $87,035 minimum to $ 95,067 maximum. Pay for this position is determined on a number of factors, including but not limited to, years and level of related experience.
