MDS Nurse

Join Our Team as an MDS Nurse!

We’re seeking a detail‑oriented, compassionate MDS Nurse (RN or LPN) to support our residents and interdisciplinary team through accurate assessment, documentation, and care planning. • Coordinate and complete the Minimum Data Set (MDS) assessments in accordance with federal and state guidelines

• Ensure timely and accurate submission of MDS documentation

• Work closely with nursing, therapy, and administrative teams to develop individualized care plans

• Monitor resident progress and update assessments as needed

• Participate in quality improvement initiatives and compliance reviews

• Educate staff on documentation standards and regulatory requirements

• Current RN or LPN license in good standing

• Prior MDS/RAI experience preferred (will train the right candidate)

• Strong organizational and analytical skills

• Knowledge of long‑term care regulations and reimbursement processes
