Hickory Grove Post Acute is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening/afternoon shift team at our Gallatin, Tennessee facility. • Provide direct patient care
  :
  Assist with activities of daily living (ADLs), administer medications, and record vital signs to ensure the health and well-being of our residents.
• Monitor patient conditions
  :
  Report any changes in patient status to the Registered Nurse (RN) or physician and assist with implementing care plans as needed.
• Collaborate with healthcare team
  :
  Work closely with RNs, physicians, and other healthcare professionals to ensure comprehensive patient care.
• Maintain accurate records
  :
  Accurately document patient information, medications, and vital signs in accordance with facility policies and procedures.
• Participate in quality improvement initiatives
  :
  Contribute to improving patient outcomes and facility processes through active participation in quality improvement activities.

Working Relationship:

• The LPN will report directly to the Registered Nurse (RN) or Director of Nursing.

• Collaborate with other healthcare professionals, including physicians, physical therapists, occupational therapists, and speech therapists.
