• Provide high-quality Licensed Practical Nurse care to patients in onsite settings.
• Assess patient conditions and document findings in electronic medical records.
• Administer medications and monitor for adverse reactions per protocol.
• Collaborate with interdisciplinary team to plan and adjust care plans.
• Ensure compliance with regulatory and safety standards, including HIPAA.
• Track and report patient outcomes to support performance measurement.
• Participate in daily huddles and maintain transparency of activity and results.
• Educate patients and families on care plans, treatments, and self-care.
• Maintain organized clinical supply management and medication inventory.
• Demonstrate initiative for professional development and skill enhancement.
• Support discharge planning and coordinate with external providers as needed.
• Adhere to a results-driven culture with performance-based recognition.

• Licensed Practical Nurse license in the applicable state.
• Provide bedside LPN care and document in electronic medical records.
• Administer medications per protocol and monitor for adverse reactions.
• Ensure HIPAA compliance and maintain patient confidentiality.
• Collaborate with interdisciplinary team to adjust care plans.
• Track patient outcomes and support performance measurement.
