- Provide high-quality Licensed Practical Nurse care to patients in onsite settings.
- Assess patient conditions and document findings in electronic medical records.
- Administer medications and monitor for adverse reactions per protocol.
- Collaborate with interdisciplinary team to plan and adjust care plans.
- Ensure compliance with regulatory and safety standards, including HIPAA.
- Track and report patient outcomes to support performance measurement.
- Participate in daily huddles and maintain transparency of activity and results.
- Educate patients and families on care plans, treatments, and self-care.
- Maintain organized clinical supply management and medication inventory.
- Demonstrate initiative for professional development and skill enhancement.
- Support discharge planning and coordinate with external providers as needed.
- Adhere to a results-driven culture with performance-based recognition.
- Licensed Practical Nurse license in the applicable state.
- Provide bedside LPN care and document in electronic medical records.
- Administer medications per protocol and monitor for adverse reactions.
- Ensure HIPAA compliance and maintain patient confidentiality.
- Collaborate with interdisciplinary team to adjust care plans.
- Track patient outcomes and support performance measurement.
